We’ve been having a look at the official report into Deepwater Horizon made to President Obama – granted, while a well blowout and STS are two very different things, lessons can still be learned.
“The immediate causes of the Macondo well blowout can be traced to a series of identifiable mistakes ……that reveal such systematic failures in risk management that they place in doubt the safety culture of the entire industry.”
Sound familiar? With regard to STS, remember the MCA’s comment on the lack of suitability of oil spill equipment? Training of crews? Environmental risk? Systematic failures in risk management? More than that, a complete failure to recognise the risks and deal with them appropriately.
